Comprehensive chronic pain management and rehabilitation for patients with nerve pain, neuropathy, mobility limitations, injuries, and functional decline.
We provide personalized, non-surgical treatment plans focused on reducing pain, improving mobility, restoring function, and supporting greater independence.
Home & Facility-Based Care
For patients with mobility limitations or difficulty traveling, we offer in-home medical visits and consultations in skilled nursing facilities (SNFs) and assisted living facilities (ALFs) when clinic visits are not feasible.
Our goal is to provide accessible physiatry, pain management, rehabilitation, and wound care where patients need it most.
Wound Care
Management
Evaluation and treatment of complex and chronic wounds, including pressure injuries, diabetic ulcers, surgical wounds, and chronic ulcers.
Wound care may include bedside debridement, advanced wound dressings, compression therapy, and coordination with home health and nursing teams to promote healing, prevent infection, and reduce complications.
Care Coordination & Treatment Planning
Development of personalized treatment and rehabilitation plans that may include therapy recommendations, medication management, and coordination of wound care supplies and dressing protocols.
We help ensure patients receive timely, coordinated care to support wound healing, recovery, mobility, and overall health.
Physician Consultation:
Specialized physiatry consultations and physical medicine and rehabilitation (PM&R) care for patients with chronic pain, mobility limitations, functional decline, and rehabilitation needs.
Our evaluations help guide personalized treatment plans and coordinated care across outpatient clinics, homes, skilled nursing facilities, and assisted living facilities.
