  • Detailed medical history and symptom pattern

  • Neurologic examination (strength, sensation, reflexes)

  • Laboratory testing for common reversible cause.


Initial lab work often includes:

  • Complete blood count

  • Comprehensive metabolic panel

  • Fasting blood glucose or A1c

  • Thyroid-stimulating hormone

  • Vitamin B12 level

  • Serum protein electrophoresis with immunofixation


Additional testing may include:

  • EMG and nerve conduction studies

  • Imaging if nerve compression is suspected

  • Referral to neurology for atypical or progressive cases



First-Line Treatment for Neuropathy



Treatment depends on the underlying cause, but initial management focuses on

symptom control and functional improvement.


For painful diabetic neuropathy, evidence-based first-line medication options include:

  • Tricyclic antidepressants (TCAs)

  • Serotonin-norepinephrine reuptake inhibitors (SNRIs)

  • Gabapentinoids (gabapentin, pregabalin)

  • Sodium channel blockers


These medications help reduce nerve-related pain.


Topical treatments may also be used, particularly for localized symptoms.

Addressing underlying causes is critical:

  • Optimizing blood sugar control

  • Replacing vitamin deficiencies

  • Adjusting contributing medications
